Adult Sponsor Application (Chrysalis Flight)

A Caterpillar must have an adult sponsor who has attended a Chrysalis Flight or Emmaus Walk. The following questions need to be answered by the adult sponsor.

A deposit of $50 is required with your application, and the balance must be paid at Registration, if not before. If you are unable to attend the Weekend that you are applying for, your deposit will be rolled over to the next available Flight date. The current cost for the Chrysalis Flight is $200. The cost for the Chrysalis Flight is not guaranteed beyond the current year.

There is also an opportunity to add a Chrysalis Flight T-Shirt for your caterpillar for $25.
